HourlyJob Summary/Purpose: The Program Coordinator - Case
Management reports to the Manager of Case Management. Under general
supervision, the Program Coordinator supports the RN Case Managers
and Social Workers in their work with the interdisciplinary
healthcare team across the continuum. The Program Coordinator
coordinates payer communication, transition plan implementation,
delivery of patient documentation and other duties in support of
case management departmental functions.Minimum Training and
Education: High school diploma or equivalent (GED) and a minimum of
five years of relevant healthcare work experience which must
include experience in patient billing required; Bachelor's degree
preferred. Proficiency in speaking, understanding, interpreting,
and writing medical terminology required. Advanced knowledge of MS
Office Suite to include Word, Excel and PowerPoint required.
Ability to speak Spanish desired. Must be personable and
comfortable interacting with medical patients and their families.
Requires excellent communication skills, attention to detail, and
ability to work independently. Demonstrates ability to collaborate
in a positive, professional manner with medical professionals.
Requires ability to work alternating weekends.Physical

recognize objects at a distance (Continuous) Ability to match or
