Job Description:
Under the general supervision of the volunteer medical director
and the Shelter Director, the Health Clinic Director is responsible
for managing the health clinic and providing quality, integrated
medical and behavioral health care to individuals in One80 Place's
shelter and housing programs. In addition to providing clinical
care, the Health Clinic Director is responsible for supervising
Healthcare Navigators to ensure collaboration with medical
providers in the community and obtain access for homeless
individuals to community-based healthcare.The Health Clinic
Director is responsible for delivering and overseeing health care
services through a trauma-informed approach and in a manner that
promotes and supports long-term housing stability. SPECIFIC
DUTIES:1. Collaborate across multidisciplinary staff to develop,
manage and directly provide an integrated care approach responsive
to the physical health, mental health, and substance use needs of
people experiencing homelessness or who are at-risk of experiencing
homelessness.2. Provides diagnoses, treat and prescribe medication
for patients with substance use disorder, psychiatric disorders and
medical disorders.3. Provide appropriate patient education that is
tailored to the individual's learning style, education, cognitive
and/or reading level, and is sensitive to their health beliefs and
homeless status. 4. Assist patients in understanding treatment
plans, and follow-up instructions. 5. Work with health clinic team
to identify social determinants of health that may be barriers to
patients' overall health and housing stability.6. Make connections
and provide appropriate referrals and follow-up. 7. Respond to
medical referrals and triage referrals based on patient acuity,
program requirements and bed availability.8. Provide guidance and
insight on the physical health, mental health, and substance use
needs of One80 Place clients to shelter staff, including Housing
Navigators, Facilities, Kitchen and Housekeeping staff members.9.
Develop and conduct trainings as needed in a variety of areas
involving mental health, physical health, substance use, access to
care, and healthcare programming. 10. Lead follow-up to incident
reports to improve safety and patient care within the shelter.11.
Provide oversight and act as a resource for point of care testing
and maintenance, medication stock inventory, and appropriate
stocking of clinical supplies, including emergency and first aid
kits throughout One80 Place facilities. 12. Work closely with the
Shelter Director and other management staff to ensure all One80
Place locations are adequately stocked and prepared for emergencies
and disasters.13. Ensure operation of medical and administrative
equipment by verifying emergency equipment availability; completing
preventive maintenance requirements; following manufacturer's
instructions; troubleshooting malfunctions; calling for repairs;
maintaining equipment inventories; evaluating new equipment and
techniques.14. Develop and manage standard operating procedures and
standing orders pertinent to the care of people experiencing
homelessness. 15. Represent One80 Place at applicable community
health care meetings and with community stakeholders involving the
planning and providing of medical services to people experiencing
homelessness. Act as point of contact for various community health
partners in coordination of care for clients. 16. Work with and
coordinate medical volunteers and medical/nursing students in
providing medical services to people experiencing homelessness.17.
Provide comprehensive documentation of all professional services
utilizing PracticeFusion, EHR and HMIS. This includes recording
patient visits, capturing demographic information, etc. 18.
Actively participate in applicable internal and external meetings
to provide insight and information on the physical health, mental
health, and substance use needs of people experiencing homelessness
or who are at risk of experiencing homelessness.19. Provide daily
supervision, regular coaching and mentoring and conduct annual
performance reviews for healthcare navigators.1. At least two
years' experience in a community health or FQHC setting and in
providing psychiatric care and chronic disease management.2. Must
have successfully completed a masters or doctoral level degree in
nursing at an accredited college or university or Successful
completion of a Board Certified, Mid-Level Provider training
program (Physician Assistant) from an accredited college or
university.3. Licensure/Certification Successful completion of the
appropriate qualifying exams as required by the State Board of
Nursing or Medical Examiners. Must be licensed as an APRN in South
Carolina or Physician's Assistant.4. Must have prescriptive
authority in South Carolina.5. Must maintain certification or
re-certification by a national certifying body recognized by the SC
Board of Nursing or Physician Assistants.6. Certifications as
required such as BLS, CPR and ACLS.7. Must have strong
communication and organizational skills.8. Competence in positive
leadership and management of staff.9. Must have a philosophy that
is consistent with the mission, vision and values of One80
Place.10. Demonstrated success in managing teams and the ability to
work effectively with others.11. Demonstrated success of mature
guidance, leadership, and flexibility.12. Demonstrated knowledge
and successful experience with electronic medical records and other
