Healthcare Social Worker - Rehab

Our Healthcare Social Workers at Bishop Gadsden, located on James Island, SC will develop and implement social service programs and policies to meet the social and emotional health needs of our residents. - Team members also advise family and care givers, provide patient education and counseling, and make referrals for other services for patients in our Rehab area of care.

Responsibilities

Develop and implement social service programs and policies to meet residents' social and emotional needs

Develop one-to-one professional relationships with residents and families to provide social support and counseling

Counsel residents and families in dealing with feelings and issues surrounding emotional, mental, environmental, or physical limitations and losses as well as end of life issues

Provide counseling for death/dying issues and/or bereavement for residents and family members as needed

Serve as the Grievance Officer for the Christie Rehab Center, investigating and following up on grievances

Coordinate family and community resources as needed to promote emotional security and work to resolve residents' problems with mental, environmental, or physical limitations

Complete required MDS assessments for all patients in the Christie Rehab Center

Facilitate the completion of all CMS required notices in the Christie Rehab Center

Participate in the admission review process for the Christie Rehab Center, assessing patients for admission

Assess and document residents' transfer or discharge needs, and participate in resident status meetings to convey level-of-care or transfer considerations to committee members

Schedule and facilitate 72-hour, care plan, and discharge meetings in the Christie Rehab Center

Collaborate with the Social Services Care Navigator, the Healthcare Social Worker, and the Apartment and Cottages Social Services Coordinator to assist with services to meet the social and emotional needs of the residents

Act as a liaison with social, health, and community agencies, making referrals for residents and families when needed including the hospice staff, home health and the consulting psychiatrist.

Serve as a resource for nurses and social work consultant in healthcare

Act as a resource and assist with training when necessary for other departments in dealing with geriatric social health and welfare concerns

Knowledgeable in current resources and topics related to the field of gerontology

Maintain open communication with the various departments (particularly nursing, resident services, dining services, and housekeeping) to ensure accurate monitoring of social/health needs of residents

Maintain high standards of professionalism, customer service, and teamwork in all interactions and relationships with residents, family members, and staff

Maintain confidentiality in records and all dealings with residents

Promote and explain residents' rights to staff and residents as necessary

Participate in community-wide activities, continuing education and training, and observe all facility policies and procedures

Perform other related duties as directed by supervisor

Qualifications

Must be able to communicate clearly and effectively and able to read, write and follow written and verbal directions in English

Ability to read and comprehend numbers and written material both on paper and on computer systems

Ability to represent the Bishop Gadsden community professionally both in manner and appearance

Must possess exceptional communication skills

Ability to interact with residents and their families as well as staff with tact and courtesy, while maintaining the confidentiality of the residents and the organization

Ability to stand, stoop, bend, reach, and lift (up to 50 pounds /-)

Required Education and Experience

Bachelor's Degree in Social Work, Counseling, or equivalent education, with relevant counseling and social services experience, preferably in a CCRC or retirement community setting

At least 5 years' experience in the gerontology field

Licensed as a Social Worker in the state of South Carolina preferred

Must be proficient in computer programs, including Word, Excel, and Outlook and able to learn other computer programs as needed
