As a Hospice Liaison at Agape Care Group, you will play a crucial role in facilitating the smooth transition of patients into hospice care. Coordinate and manage referrals from hospitals, physicians, and other healthcare professionals. Educate patients and their families about hospice services and the benefits of palliative care. Conduct initial assessments to determine patient eligibility and necessary care plans. Build and maintain strong relationships with healthcare providers and community organizations. Assist in the development and implementation of patient care policies and procedures. Ensure compliance with all federal, state, and local regulations concerning hospice care. Strong interpersonal and communication skills Ability to manage sensitive information Extensive knowledge of hospice care regulations and guidelines Proficiency in patient assessment and care planning Strong organizational and time-management skills Medical Insurance Dental Insurance Vision Insurance 401k Current RN license CPR certification Certified Hospice and Palliative Nurse (CHPN) preferred
