plus bonuses We are seeking a dedicated Wellness Director (LPN or
RN) to join our Assisted Living and Memory Care community. This
role is crucial in overseeing the health, well-being, and overall
care of our residents. The Wellness Director will be responsible
for the daily management, coordination, and delivery of resident
care, including leading the clinical team, ensuring quality
standards, and maintaining regulatory compliance. Key
Responsibilities: Coordinate and manage residents' health and
wellness needs, including medication administration, health
assessments, and ancillary health services. Oversee the medication
program and ensure regulatory compliance with medication
administration procedures. Conduct regular assessments, update
Individual Service Plans (ISPs), and coordinate family meetings as
needed. Manage and train the clinical team to ensure optimal
resident care. Maintain care plans and ensure all staff are
properly trained and compliant with state and federal regulations.
Ensure compliance with all SC Department of Health and
Environmental Control (DHEC) regulations and company policies.
Maintain a high standard of care through ongoing monitoring and
quality assurance initiatives. Schedule and manage staff shifts in
compliance with DHEC requirements. Engage with residents, families,
and staff to maintain positive relationships and a community
atmosphere. Qualifications: Active LPN or RN license in the state
of South Carolina. Minimum of 3 years of experience in a wellness
or clinical coordination role. At least 2 years of leadership or
supervisory experience in a healthcare or assisted living setting
Strong knowledge of SC DHEC regulations and assisted living
standards. Experience with memory care, assisted living, or
long-term care is highly desirable. Proficiency in medication
administration and care plan development. Certification to
administer medications and demonstrate competence in assessment,
medication administration, and triage. Ability to work flexible
hours, including days, evenings, weekends, and holidays if needed.
Successful completion of drug testing and criminal background
checks upon hire and upon request.
