Manager will be responsible for coordinating patient care and
ensuring effective disposition planning within the Emergency
Department of a major hospital system. Responsibilities include:
Conducting comprehensive assessments of patients' medical,
psychosocial, and discharge needs. Developing individualized care
plans and disposition plans in collaboration with the healthcare
team. Coordinating and facilitating patient discharge planning,
including arranging post-discharge services and follow-up care.
Communicating effectively with patients, families, and healthcare
providers to ensure understanding and adherence to care plans.
Monitoring patient progress and adjust care plans as necessary to
meet changing needs. Advocating for patients' needs and
preferences, ensuring patient-centered care. Qualifications: Active
Compact or SC RN license Bachelor's degree in nursing (BSN) Active
BLS certification (AHA) 3 years of clinical nursing experience,
with 1 years in the Emergency Department (or similar department -
ICU, Step-down, Surgical post-op) Excellent communication,
organizational, and problem-solving skills. Ability to work
effectively in a fast-paced, multidisciplinary environment .
