teams collaboratively manage our complex patient panels. These
teams are led by Physicians, Nurse Practitioners, and Physician
Assistants, with supporting care provided by RN Nurse Care
Managers, Social Workers, Pharmacists, Behavioral Health, and other

communities.The Outreach Support Specialist role is a critical
member of the Landmark Local Outreach Team. This role reports to a
supervisor or the Director of Community Outreach and Patient
Engagement. Responsibilities are to ensure a positive patient
experience with Landmark, as well as to build good relationships
with our local outreach team to ensure that Landmark is a reliable
and trusted partner in patient care.This position is full - time,
Monday - Friday. Employees are required to work our normal business
hours of 8:00 AM - 5:00 PM EST (Eastern Standard Time zone).We
offer 5 - 7 weeks of training. Training will be conducted virtually
from your home.*All Telecommuters will be required to adhere to
UnitedHealth Group's Telecommuter Policy.Primary
Responsibilities:
- Regular contact with patients in making reminder calls for
upcoming appointments
- Support patients in scheduling home visits
- Support Landmark Local Outreach Team with operational tasks and
reporting of engagement
- Handle escalated patient issues, ensuring the appropriate
resources are involved
- Route voicemail messages and emails received to appropriate
individual and team
- Manage and execute current processes established for Local
Outreach Team
- Other duties may be assigned
- The position is home - based
- Reports directly to the Supervisor or the Director of Community
Outreach and Patient EngagementYou'll be rewarded and recognized

- High School Diploma / GED (or higher) OR equivalent years of
work experience
- Basic proficiency with using Microsoft Office applications:
Microsoft Word, Microsoft Excel, and Microsoft Outlook
- Experience with escalated customer service issue
resolution
- Ability to work any shift between the hours of 8:00 AM - 5:00
PM EST (Eastern Standard Time zone) from Monday - FridayPreferred
Qualifications:
- 1+ years of experience with handling inbound and outbound
calls
- Experience as a Medical Assistant, Pharmacy Tech, OR other
clinical or administrative role, preferably in a health care
settingTelecommuting Requirements:
- Reside within a 1-hour commutable distance of Charleston,
Myrtle Beach, Greenville, OR Columbia, SC
- Required to have a dedicated work area established that is
separated from other living areas and provides information
privacy
- Ability to keep all company sensitive documents secure (if
applicable)
- Must live in a location that can receive a UnitedHealth Group
approved high-speed internet connection or leverage an existing
